Another purpose of ACos is to reduce readmissions and
consumption, according to Hoyt. “You are accountable
for the care, and you’re getting a per member, per month
fee or certain expense target amounts, so you have to
reduce consumption,” he says.
For example, when a patient arrives at the hospital to be
admitted, often the hospital will want to run lab tests,

even if the patient had the same tests done earlier by the
physician’s office. “Why re-run them? Having an HIE is a
way to reduce consumption, and those organizations that
have their own private HIE, they have data that shows a 7
to 12 percent reduction in ancillary consumption per
patient per year because they have data all in one system,”
says Hoyt. “No one in the ACo is going to be on one
EHR vendor, so you need an HIE.”
DHIN’s long-term vision is to be that trusted source of
information not only for hospitals and physicians but
health plans, employers and consumers. Its goal is to
integrate clinical, financial and administrative data for
more informed decision making across a broad range
of use cases so that public health and policy setting
can be done more intelligently with the ability to see
population-level data. “organizations that are interested in setting up an ACo or patient-centered medical home certainly have some needs in this area,” says
Lee. “In the future, maybe we can add claims data to
the clinical data that would improve decision making
at all kinds of levels.”
